Chronic Care Management

Chronic Care Management supports patients with two or more chronic conditions. Some patients need more support to manage their chronic conditions, leading to a better quality of life.

Our RN-led Care Team, under your general supervision, engages the patient to reinforce the care plan you have already established. Among other things, the Team helps to increase medication compliance, lifestyle modification, and patient education.​

While RPM gathers patient physiological health data, our Chronic Care Management service provides a trained member of our RN-led team to communicate with patients directly, supporting them as long as they suffer from chronic illnesses.
